Causes of iron deficiency anemia

Iron deficiency is one of the most common conditions I evaluate and treat at Mountain View Primary Care Medical Clinic. Many of my patients are surprised to learn they have it, because the symptoms develop so gradually that they often go unnoticed for months or even years.


I wrote this to help you better understand iron deficiency, what causes it, how it's diagnosed, and what treatment looks like. Whether you're experiencing symptoms or simply want to learn more, I hope you find this information helpful.


What Is Iron Deficiency Anemia?



Iron is a mineral your body needs to produce hemoglobin, the protein in red blood cells responsible for carrying oxygen throughout your body. When iron levels become too low, your body cannot make enough healthy red blood cells, and iron deficiency anemia develops.


This is one of the most common nutritional deficiencies I encounter at Mountain View Primary Care Medical Clinic.


Recognizing the Symptoms



Iron deficiency can be easy to overlook because the symptoms tend to appear gradually. Many of my patients initially attribute how they feel to stress or a busy schedule. However, the following symptoms are worth paying attention to:


- Persistent fatigue and low energy, the most common concern my patients report


- Difficulty concentrating or mental fogginess


- Shortness of breath, particularly with physical activity


- Dizziness or lightheadedness


- Pale skin


- Hair loss


- Brittle or easily broken nails


- Restless legs, especially at night


- Unusual cravings for ice, dirt, or starch, a condition known as "pica"


- Irritability or changes in mood

Common Causes of Iron Deficiency



In my experience, iron deficiency typically falls into one of several categories:


Blood Loss


This is the most frequent cause of iron deficiency anemia in adults. Sources of blood loss include:


- Heavy menstrual periods


- Gastrointestinal bleeding from ulcers, polyps, hemorrhoids, or more serious conditions such as colon cancer


- Frequent blood donation


- Long-term use of medications such as ibuprofen or aspirin, which can cause gradual stomach bleeding


Poor Iron Absorption


In some cases, the body has difficulty absorbing iron from food. Conditions that can interfere with absorption include:


- Celiac disease


- Helicobacter pylori (H. pylori) stomach infections


- Prior bariatric (weight-loss) surgery


- Chronic use of acid-reducing medications such as omeprazole


Insufficient Dietary Iron


Vegetarian and vegan diets, or diets that are generally low in iron-rich foods, can lead to gradually declining iron levels.


Increased Iron Demands


Pregnancy significantly increases the body's need for iron, particularly during the second and third trimesters.


Why the Evaluation Differs for Women and Men



Understanding the differences between how iron deficiency presents in women versus men is an important part of how I approach each patient's care at Mountain View Primary Care Medical Clinic.


Women of reproductive age are more likely to develop iron deficiency anemia, most commonly due to monthly menstrual blood loss. In many cases, the cause is apparent, and we can begin treatment promptly while also addressing the underlying bleeding.


Men and postmenopausal women require a more thorough investigation. Without menstrual blood loss as an explanation, iron deficiency in these groups raises concern for gastrointestinal bleeding, which can sometimes indicate serious conditions such as colon cancer. Current medical guidelines recommend that these patients undergo both a colonoscopy and an upper endoscopy to identify any source of bleeding. I understand that this can feel concerning, but it is an essential step in ensuring nothing is missed.


How Iron Deficiency Is Diagnosed



At my clinic, I take a thorough, evidence-based approach to diagnosis:


1. Initial blood work. I begin with a complete blood count (CBC) and iron studies. The most valuable test is your ferritin level, which reflects how much iron your body has stored.


2. Accounting for other conditions. For patients with chronic illnesses such as kidney disease, heart failure, or inflammatory bowel disease, I may order additional markers. Inflammation can cause ferritin levels to appear falsely normal, so a more complete picture is sometimes needed.


3. Identifying the underlying cause. Once iron deficiency is confirmed, the next priority is determining why it developed. Depending on your age, sex, and medical history, this may include testing for H. pylori, screening for celiac disease, or referring for endoscopic and colonoscopic evaluation amongst other evaluations.


Treatment Options



The encouraging news is that iron deficiency anemia responds very well to treatment. Here is what a typical treatment plan looks like:


Addressing the Root Cause


Before focusing on iron replacement, it is important to treat the underlying reason for the deficiency, whether that involves managing heavy menstrual bleeding, treating a stomach ulcer, or addressing celiac disease.


Oral Iron Supplementation


For most patients, oral iron is the recommended first step. Here are some important guidelines I share with my patients:


- Ferrous sulfate (325 mg) taken once daily or every other day is the standard approach. Research has shown that every-other-day dosing can improve absorption and reduce side effects.


- Taking iron on an empty stomach enhances absorption, though it is not always well tolerated.


- Vitamin C, such as a glass of orange juice taken with your supplement, can significantly improve iron absorption.


- Coffee, tea, and calcium-rich foods should be avoided within an hour of taking iron, as they can interfere with absorption.


- Side effects such as constipation, nausea, and stomach discomfort are common. If you experience these, please contact your primary care office so they can adjust your plan rather than discontinuing treatment.


Intravenous (IV) Iron


In certain situations, iron delivered directly into the bloodstream through an IV is the better option. IV iron may be recommended if you:


- Cannot tolerate oral iron due to side effects


- Have a condition that impairs iron absorption, such as celiac disease or prior bariatric surgery


- Are managing a chronic condition such as heart failure, kidney disease, or inflammatory bowel disease


- Are in the second or third trimester of pregnancy


- Have ongoing blood loss that exceeds what oral supplementation can replenish


Modern IV iron formulations have an excellent safety profile. Serious reactions are rare, and many of my patients have benefited greatly from this approach.


Follow-Up Care


Blood work is typically checked at least 4 weeks after initiating treatment to confirm that iron and hemoglobin levels are improving. If progress is not where it should be, further evaluation may be warranted.


Supporting Your Iron Levels Through Diet



While diet alone is generally not sufficient to correct iron deficiency anemia, incorporating iron-rich foods plays an important role in maintaining healthy levels over time:


- Red meat, poultry, and fish, the most readily absorbed sources of dietary iron


- Beans, lentils, and tofu


- Dark leafy greens such as spinach and kale


- Fortified cereals and breads


- Dried fruits such as raisins and apricots


Pairing plant-based iron sources with vitamin C-rich foods, such as bell peppers, tomatoes, or citrus fruit, can help enhance absorption.

Q: Why is every-other-day dosing recommended?


When you take a dose of iron, your body produces a hormone called hepcidin that temporarily reduces iron absorption for up to 48 hours. Every-other-day dosing works with this natural process, allowing for improved absorption and fewer gastrointestinal side effects.


Q: What can I do if oral iron causes stomach discomfort?


Gastrointestinal side effects are very common with iron supplements. Strategies that may help include switching to every-other-day dosing, taking the supplement with a small meal, or trying a different iron formulation. If oral iron remains poorly tolerated, IV iron is a safe and effective alternative. 


Q: Is IV iron safe?


Yes. Modern IV iron formulations are well established and have an excellent safety record. Serious allergic reactions are rare. Many of my patients who were unable to tolerate oral iron have had very positive experiences with IV iron therapy.


Q: How long does treatment take to work?


Fully restoring iron stores can take several months. It is important to continue treatment for the full recommended duration, even once you begin feeling better.


Q: Why might an endoscopy or colonoscopy be recommended?


In men and postmenopausal women, iron deficiency anemia can indicate slow, often undetectable bleeding in the gastrointestinal tract, sometimes from polyps or colon cancer. A colonoscopy and upper endoscopy allow for a thorough evaluation to ensure no underlying cause is overlooked. Early detection in these situations can make a meaningful difference.


Q: Can iron deficiency anemia recur after treatment?


Yes, it can return if the underlying cause has not been fully resolved. This is why ongoing follow-up is so important. Regular monitoring allows us to identify any recurrence early and intervene before symptoms return.
